Users assigned with the MAR Configuration Approve role are able to create and approve new Medication Administration Records (MARs). To create a new Medication Administration Record for an individual, you may follow the steps below:

  1. From your Dashboard, click on the New link beside the ‘Configuration’ option under the Health Tab.




  2. Select the appropriate program from the ‘Select Program’ page.







  3. On the ‘MAR Configuration Selection’ page, select the appropriate individual, month and year from the respective fields and click on the Submit button.







  4. The ‘Medication Administration Record Configuration’ page will be displayed for the month that has been selected in the ‘MAR Configuration Selection’ page. Here, all the active medications that have been prescribed for the individual will be displayed. You will be able to add new medications from the MAR as well.


    Users can add multiple schedules for the same medication. Data can only be recorded for the schedule that is active in the current month.







    • The multiple schedules that are set in a Medication History form will appear accordingly under the corresponding medication name on the MAR.


    • Under the medication names in the MAR, the Strength field will show data from both Strength and Strength Unit of the Medication History form and the Give Amount/Quantity field will show data from the Give Amount/Quantity and Measurement Unit of the Medication History form.


    • The data entered in the ‘Administration Details’ section of the Medication History such as Begin Date & Time, End Date and Time, Frequency, Schedule will also appear on the MAR. The Schedule Repeat field will show data from the ‘Schedule Interval’ sub-section of the Medication History form. The Schedule Time Slot(s) field will show data from the ‘Schedule Time(s)’ sub-section of the Medication History form.


    • In case of medications that have multiple schedules, the MAR will show any past and/or future schedules if there is a scheduled medication or treatment for the current month.


    • The grid to record data on the MAR appear gray when a medication is not scheduled for the current month. The grids on the past and future schedule above appear gray as the schedule is not active for that particular time period.


Data previously entered in the Dose and Total Dose fields in the Medication History form before the 2016.1.0 Release (5/14/2016) will show in the Give Amount/Quantity field on the MAR as [Dose: x Total Dose: y].


  1. The ‘Indication/Purpose’ of each medication provided in the Medication History form will appear in the MAR under that corresponding medication to help staff understand the purpose of the medication they are administering. The Instruction/Comments entered in the Medication History form will also appear on the MAR under the Indication/Purpose field.

  2. You may click on the View this form for data entry, Add New Medication, and Medication Details links as follows.

    • You may click on the Add New Medication link if you want to create a new Medication History form for the individual straight from an MAR form. The Program Name and Individual Name will be auto-populated from the MAR form.

    • If an individual does not have any current medication then click on the Create New Medication History form link.

    • If you would like to continue directly to the MAR form for entering data, you may click on the View this form for data entry link and follow the steps mentioned here.



  3. If you need to view the existing Medication History form then click on the Medication Details link.

If you need to make changes to an approved Medication History (changing the dosage, begin and end dates or any other change) we suggest users to make the changes after copying the existing the Medication History form and discontinuing the older form. For more information, please click here.

  1. Clicking on the Medication Details link will take you to the Medication History page of the individual. You will be able to make changes in the Medication History form if it has not already been approved. Clicking on the Drug Details link will provide you the detailed information of that particular medication and its side effects in a ‘Drug Details’ pop-up window.

  2. After making the required changes, click on the Submit button to save the changes and this will take you to the ‘Medication Administration Record Configuration’ page. If you want to Discontinue the medication then click on the Discontinue button.

  3. In the ‘Medication Administration Record Configuration’ page, approve the form for Data Entry by clicking on the Approve button located at the bottom of the page.

  4. Once you click on the Approve button, a success message will be displayed informing that the Medication Administration Record has been ‘Successfully Approved’. You may view the form for data entry by clicking on the View this form for data entry link or you may open the form in configuration mode by clicking on the Open This Form in Configuration Mode link.

  5. The current MAR gets locked whenever changes are made to a medication (for example adding End Date, discontinuing a medication or adding a new medication). Changed medications are highlighted in pink in the MAR form and a message will appear on top of the form saying that it requires to be unlocked. The screenshot below shows that the End Date was added to the medication which had locked the MAR form.

A locked MAR form requires that a user with the MAR Configuration Approve role approve the MAR to unlock it for data entry.

  1. The medication will be highlighted in pink with the ‘Last Updated’ date mentioned in the previous months’ approved MAR for the individual.

  2. Once a MAR has been approved, you will be able to start recording data on the form. For details on entering data in the MAR form, please refer to the Entering data in MAR user guide.
