Individual Service Plan (ISP)

In Therap 2012.0, the default view of the approved forms of the ISP Program, ISP Plan, Hab Plan and Individual Plan of Protective Oversight and Safeguards (IPOP) modules have been redesigned to enhance the process of archiving each of the changes made to an approved form.

The feature of archiving static documents of the approved forms will be applicable with the approval of the new forms (not applicable for the existing approved/updated forms). To implement this feature for the existing forms, users will need to discontinue and copy the existing forms and then approve the newly saved draft forms.
  • An approved form will now have two different version - one is 'Static Document' that reflects the exact view of the form when it was originally approved or last updated and another one is 'Dynamic Document' that will display the form with the current dynamic data (external information that has changed in the system after the last approve/update such as, Shared Contacts, Individual Contacts, IPOP or the ISP Programs linked within a Hab Plan or ISP Plan).



  • When a form is originally approved, a static version of this form will be saved with the 'Archived (Approved)' status and will be archived as the 'First Approved (Static) Document'. This Approved (Static) Document can be accessed by clicking on the Open First Approved (Static) Document link located at the top of the default Last Updated (Static) Document.

  • In order to edit an approved form, users will need to click on the click here link at the top of the form, or the 'Show Dynamic Document' button at the bottom of the form. This will direct them to the Dynamic Document and allow them to edit the form along with the dynamic data.

  • After updating the approved form, a new static document will be created with the 'Approved (Updated)' status and will be archived as the 'Last Updated (Static) Document'. This static document will become the default view for the approved document. Users will find all the archived static documents by clicking on the Update History link located at the top of the form.



  • In the static document (for both First Approved/Last Updated (Static) Documents), any external links (such as, links to the approved IPOP and ISP Program forms) will direct users to the static document of the IPOP/ISP Program that was available at the time of approval/update of the form.

    For example, at the time of approving or updating an ISP Plan, if the linked IPOP form has its 3rd updated static document then the system will archive the First Approved/Last Updated (Static) Document of the ISP Plan with the associated (3rd) updated static document of the IPOP form. After approving the ISP Plan, clicking on the IPOP link located on the First Approved/Last Updated (Static) Document will direct users to the associated static document of the IPOP form (not the currently available static document of the First Approved/Last Updated IPOP form).



  • In case of the Dynamic Document of the approved form, any external links (such as links to the approved IPOP and ISP Program forms) will direct users to the Dynamic Document with the 'Approved (Dynamic)' status of the IPOP/ISP Program form.

  • Users who approve/update an approved form, their First Name, Last Name and Title will be retained for the following forms: IPOP, ISP Plan, ISP Program, ISP Data, Hab Plan and Custom Forms. The 'Approved By' area of the approved form will display the approver's original name and title that was available in the system at the time of First approval of the form. If the same user updates the approved form when his/her name or title is changed in the system, then The 'Last Updated By' field will reflect user's changed name and title.

    Individual's original First Name, Last Name and ID will also be retained in the IPOP, ISP Program, ISP Plan, ISP Data, Hab Plan and Custom forms even if their names are changed.

ISP Program

  • While creating a new ISP Program, users will now be able to configure the ISP Program as billable. We have included a new Is Billable ISP Program field in the ISP Program where users will be able to select/deselect the 'Yes' or 'No' options. This option is only applicable for the 'Non-Grid' type of ISP Program.

    If users select the 'Yes' option in the Is Billable ISP Program field, then while collecting ISP Data for this ISP Program, the Billable field will automatically be selected as 'Yes' in the ISP Data Collection page.

Habilitation Plan

  • Users will now be able to copy a Habilitation Plan for an Individual.



  • While updating an approved Hab Plan, users will now be able to include significant changes along with respective review date. A new option - 'Significant Change' has been added in the Summary Type field under the Update Summary section. Users can also enter the Review Date applicable for the significant changes made to the approved Hab Plan.



    Similar to the Six Month Review Date, the date entered for Significant Change will later be displayed as the Last Revision Date under the General Information section of the Hab Plan.

  • Users will now find a new link to print the PDF Document of the Habilitation Plan along with a signature sheet. The new link Display PDF (with Signature Sheet) has been added at the bottom of the Habilitation form. This printable PDF Document contains a separate page for signatures.

Individual Plan of Protective Oversight and Safeguards

  • Users will now be able to copy an approved Individual Plan of Protective Oversight and Safeguards (IPOP) form and save it as a draft for the Individual.

ISP Data

  • The 'Scoring Comments' column has been added to the Detailed Excel document that is exported for the ISP Data search results. This column will display the comments entered for each of the score. The data for the 'Comment' field of the ISP Data Collection page, will only be available in the Excel document exported by clicking on the 'Export to Excel' link.

Individual Data

  • The following fields in Individual Data can now only be edited/updated by users with the Individual Admit/Discharge Role or the Individual Administrative Role:
    Name of Individual Social Security Number (SSN)
    Date of Birth (DoB) Gender
    Medicaid Medicare
    Insurance Admission
    Discharge Death Date
    Individual Time Zone ID Types and Numbers



  • The 'Mental Retardation' field on the Individual Data form has been renamed to 'Intellectual Disability'. Users may now deselect the option and leave this field blank.



  • The option 'Eastern Orthodox' has been added under Religion.



  • The Race field now has more items in the list. Also, Users will now be able to add multiple items from the Race list by clicking on '+' and may remove items by clicking on '-'.



  • The Gender type 'Unknown' has been added to the 'Gender' drop down list.



  • The 'Citizenship' list now contains the option 'Canada'.



  • A 'Suffix' field has been added to the form where users can add suffixes like Sr., Jr., I, II, III, etc.



  • The 'Language' field has been renamed to 'Primary Oral Language'.



  • A new field named 'Primary Written Language' has been included.



  • A field named 'Interpreter Needed' has been added with the options 'Yes', 'No' and 'Unknown'.



  • 'Hispanic' is now an option for the 'Ethnicity' drop-down.



  • Users will now be able to record Marital Status Date.



  • 'Attention or in care of' field has been added to both 'Residential Address' and 'Mailing Address' sections.

    The 'Phone' field has been renamed to 'Primary Phone' and an option for 'Secondary Phone' has been added. We have also included an 'Additional Phone' field. This field may contain multiple phone numbers.



  • A 'Country' field has been added within 'Residential Address' and 'Mailing Address' sections.



  • In the Add New Contact page, "Attention or in care of", "Additional Phone", "E-mail" and "Web Address" fields have been included. Also, a 'Mailing Address' section has been added to this page.

  • Users will now be able to access Pending Admission Notes link located at the bottom of the 'Individual Data' page. They will be able to view the 'Pending Admission Notes' that was entered when the Individual was in 'Pending Admission' status.

  • While importing Individuals using Excel, the maximum number of Individuals per import has been set to 1000 Individuals.



    A warning message will appear if the users try to import more than 1000 Individuals from Excel at a time.



    The Sample Excel file for importing Individuals has been updated to accommodate the changes made to Individual Data, users will need to download the sample Excel by clicking on the Download a sample Excel file (with supported format and help) link in order to import Individuals to the system.

Shared Contacts

  • In the Create/Update Shared Contact page, "Address" and "Mailing Address" sections have been added for users to record more information regarding the Shared Contact.

    'Attention or in care of' and 'Country' fields have been included in both "Address" and "Mailing Address" sections. Users will now be able to select from a number of countries included in the drop down list. Please note that "State" is required if user selects 'Country' as "USA" or "Canada".

    The 'Phone' field has been renamed to 'Primary Phone' and an option for 'Secondary Phone' has been added. We have also included an 'Additional Phone' field in both 'Address' and 'Mailing Address' sections. This field may contain multiple phone numbers along with other descriptions.

    'E-mail' and 'Web Address' fields are now available under the "Address" section.

Drug-Allergy and Drug-Drug Interaction

Drug-Allergy interaction information will now be displayed on both the Allergy Profile and Medication History page.

  • When a new allergy is added, it will be checked against active medications and a warning message will be displayed for adverse interaction.



  • If any active allergy has an interaction with any active medication, then this information will be displayed in the Allergy Profile page.



  • Drug-Allergy and Drug-Drug interaction information will be shown in Medication History page.

T-Task

The T-Task module has been upgraded with the following new features:

  • Users will now have the option to assign a task to a particular user of a Program. They can select the appropriate user from the Assign To drop-down menu located just below the Program drop-down menu.



  • Users may now re-assign already assigned tasks to another user, by editing the task.



  • Task assignment information has been enhanced. Users will find the name of the user who assigned the task and the time of the task assignment beside the ‘Assigned By’ and the ‘Assigned At’ area.

    If a task has been reassigned, users will also find the information for the previous assignment beside the ‘Previously Assigned To’ area.



  • Users will now have the option to deny a task that is assigned to him/her. The 'Deny' option appears on clicking the (X) button located at the top right area of the task.



    *Users will be able to remove a task (not assigned to him/her) from his/her list. Clicking on the (X) will display the Remove From My List link. This will remove the task from the user’s list only and will be available to other users having access to the Task.



  • Users can now search for tasks using parameters such as ‘Assigned User’, ‘Title’ and ‘Due Date’. They can access this search option by clicking on the Search link available in the T-Task area on the FirstPage or the Dashboard. They can also search tasks by clicking on the Search Task link located at the bottom right corner of the T-Tasks page.





  • Users can now view Activity Tracking of the T-Task module for the ‘Add’ , ‘Assign’, ‘Deny’, ‘Delete’, 'Update', 'Done' and 'Pending' actions.

General Event Reports

GER updates for Users in Nebraska
  • The option 'Other' has been removed from the 'Injury Type' and 'Injury Cause' drop-downs under the Event 'Injury', 'Medication Error Type' drop-down under the Event 'Medication Error' and 'Event Type' drop-down under the Event 'Other' for Nebraska specific GERs only.
  • 'Describe what happened before the event' and two other follow-up fields 'Corrective Action Taken' and 'Plan of Future Corrective Action' have been made required.

GER Updates
  • Oversight Agency Users will be able to view GERs for the last fifteen days of all the Individuals in all the Linked Providers in their Caseloads from their Unified Dashboard using the 'View' option under the General Event Reports module. Previously, this option used to show only the GERs that were submitted by the Oversight Agency Users.
  • User without GER Abuse/Neglect enabled for his/her User account will not be able to Approve Abuse/Neglect specific GERs created by him. This user will only be able to View and Submit those GERs if he/she has GER View and GER Submit roles. Previously, he/she could Review/Approve an Abuse/Neglect specific GER if it was at In-Prep/Returned states and if he/she was the submitter.

New Modules to meet CCHIT Requirements

Therap is pursuing CCHIT certification as a Long Term and Post Acute Care (LTPAC) EHR. The 2012.0 Release introduces several modules to meet CCHIT requirements. For more information on Therap's CCHIT compliance plan, please refer to the CCHIT Requirements page.

Medication Profile New!

Medication Profile has been introduced to display an Individual's current medications, Home medications, Review of medications and Reconciliations in a single page. Users with Medical Information View/Medical Information Edit Role will have access to it.

  • Individual's Current Medication List, Home Medications, Review of Medications and Reconciliations will be displayed in his Medication Profile and users will be able to navigate to the corresponding modules from here. Users can also add a comment on the Medication Profile.
  • If an Individual does not have any medication information available then users will get a 'Takes no Medication' field for that Individual to mention explicitly whether s/he is on 'no medication'. This is a CCHIT requirement.
  • Users will also be able to view the PDF of the Medication Profile.

Medication Reconciliation New!

To indicate that medication reconciliation has been completed at admission, discharge and each change in level of care during the facility stay, Therap now provides the ability to capture a signature or the User ID and the date stamp as per the CCHIT requirements. Users with the Medical Information View/Medical Information Edit Role will have access to the Medication Reconciliation of Individuals.

  • Reconciliation Type can be Admission, Discharge, Change in Level of Care, Transfer or Other.



  • There will be two drop-down menus for selecting the reconciler. One is the User List and the other one will be the Physician and Registered Nurse list from Contact.
  • There will be a date range to define for which period this reconciliation is active.



  • Users will be able to attach a file for each reconciliation.
  • Users will also be able to include comments for the reconciliation.

Home Medication New!

According to the CCHIT requirements, Home Medications can now be recorded and displayed in Therap Applications.

  • A 'Home Medication' check-box has been added in the Medication History section under the Health Tracking module to record the medication as 'Home Medication'.



  • The medications displayed from 'Medication Review' can be filtered by Home Medication.

Home Medication
A "Home Medication" is any prescription or over the counter medication that is owned by the Individual patient and brought into a facility such as a respite center, nursing home, hospital, etc, that were not ordered for that person by an attending physician or other prescriber on duty at that facility.

Individual History New!

The Individual history Module will not be readily available with this release. Please contact Therap to enable this module for your agency.

The 'Individual History' module has been introduced to document and manage Individual's Historical Data. Here, users with Medical Information Edit/ Medical Information View Roles will now be able to record the Current Illness/Chief Complaint of an Individual along with History of Present Illness, Past Medical History, Social History, Family History, Assessments and Plans, Functional Status, General Status, Review of system, HEENT Exam, Other Physical Findings and Annotations of an Individual.
Information on Vital Signs will be pulled from the 'Vital Signs' section under the Health Tracking module and will be displayed under Individual History.



Medications, Diagnoses, Allergies, Infections and Immunizations of an Individual can also be looked up from this module. Users will be able to view the Individual History in PDF format from the 'Print PDF' option and can also add external attachments to the form.

Individual Care Plan New!

The Care Plan Module will not be readily available with this release. Please contact Therap to enable this module for your agency.

This new module will allow agencies and care providers to better document Individualized Care Plans. As care requirements and planning are Individual specific, this module allows agencies to create care plans for each Individual. Furthermore, users are able to upload Care Plans to the Global Template Library and import Approved Care Plans created by users of other agencies.

  • Users will be able to create a new Individual Care Plan and add Approaches and Goals to the Plan.
  • Users will be able to add Review Comments and Team Meeting Notes.
  • Care Plans can be discontinued once goals are achieved.



  • A new Global Care Plan Template Library Role has been introduced. With this Role, Users will be able to create and publish Care Plan Templates for approval.
  • Once a Care Plan Template is approved, it will be added to the global template library under 'Global Care Plan Template Library'.
  • The Author of the Care Plan Template will be notified when his/her Care Plan Template is published.

Physician InformationNew!

  • Administrators or Users with the User Administrative Role will be able to add NPI Number, UPIN Number, DEA Number and License Number for Physician users. These identifiers are for physicians who are users of Therap.



  • Physicians can be quickly looked up from the Physician List under the Physician Information section in the Admin mode FirstPage. This list can also be exported to Excel.

General Updates

  • Country field has been included in the Individual Data page in "Residential Address" and "Mailing Address" sections, in the Create and Update Shared Contact page under "Address" and "Mailing Address" sections, in General Event Reports (GER) forms in the "Event Information" section. Users with proper Administrative Roles will be able to select from a list of countries from the drop down menu in the Country field in the Create New Site forms and Personal Finance forms.

    The drop down list now includes a list of names of Countries. State field is required if "USA" or "Canada" is selected from the drop down menu.



  • More Provinces in Canada have been included in the 'State' drop-down menu.



  • User with Individual Data View Role can view Approved IDF Extension Forms.

  • In the "Medicaid Service Coordination" Notes form, "Outcomes of an ISP review meeting and other relevant information" area has been included in the 'ISP Review' section. Previously, it was in the 'Monthly Summary' section.



  • Users will now be able to copy an approved custom form (such as Medication Administration Assessment Tool, Risk Management Assessment and Plan, etc) and save it as a draft for the Individual.

LiveHelp Upgrade

We have upgraded Live Help in Therap 2012.0 and moved to LivePerson for this service. All Live chat conversations will still be fully encrypted and transferred in a completely HIPAA compliant manner as before. They will now go directly into My Issues, users will not have to use the 'Send via SComm' option. Also, users will now be able to request us to share their screen at any time of the day.

In case of Offline Messages, we strongly recommend our users to not include any Protected Health Information (PHI) within the content as these will be transferred over non-secure media.

New York Specific Updates

IRMA - GER State Specific FormNew!

A New York state specific form named 'Form IRMA' has been added in the State Specific Information section of GERs. Here a user can record Incident Details, Initial Findings and Physical Findings for an Individual. They may choose the Disposition status and enter the Special Review Committee Minutes in this form. Once an IRMA form is added to a GER, users will be able to export that form in XML format and submit it to the 'Incident Report and Management Application' website.

To view IRMA forms, users should be assigned with GER Followup or GER Approve Roles.
If the GER form is with the Pending Approval or Approved status, then IRMA form can be added to the GER.
  • The TABS ID field in the IRMA form pulls data (if available) from the 'TABS ID' field which has been added to the 'IDF Extension Form' under the 'ISP and Habilitation Plan' section on the FirstPage.





  • The Program TABS ID field in the IRMA form pulls data from the 'Program TABS ID' field which has been added to the 'Create New Program' page and 'Update/Delete Program' pages under the 'Program/Site' section on the FirstPage of Admin mode. Users with proper Administrative Roles will be able to enter data in the 'Program TABS ID' field.



  • Administrators or Users with Provider Setup Administrative Role will be able to upload IRMA Agency Committee Info in .CSV format to system from the Provider Preference section.

Hab Checklist
  • Users in New York will now be able to select multiple Hab Plans (both discontinued and approved Hab Plans) of the same type of service (such as, Group Day Hab) while generating the Hab Checklist.



    The Hab Checklist will then display the cumulative results of the valued outcomes for both discontinued and approved Hab Plans, depending on the ISP Data or Attendance Data of the month.
Habilitation Plan
  • Users will be able to select from a list of Valued Outcomes in order to add them to Habilitation Plans. For example, a HCB Waiver Service of type 'IRA Residential Services' is added to an ISP Plan of an Individual where the associated Valued Outcome(s) are selected. Next, when you create a new Habilitation Plan of 'IRA Residential Services' Hab type for that particular Individual, the associated Valued Outcome(s) will appear in the Valued Outcome(s) list from where it has to be selected in order to add them to the associated Hab Plan.

Billing Checklist Report
  • Users will now be able to generate the Billing Checklist Report without completing the IDF Extension form first.

Comprehensive Health Assessment Tool (eCHAT)

eCHAT Search for State Providers
  • Users of State Providers with Comprehensive Health Assessment Tool (eCHAT) enabled, will be able to access eCHATs of Individuals present in their Caseload with matching Oversight ID in different Linked Providers. Users will be able to access this feature from their Unified Dashboard using the 'Search' option of the 'Comprehensive Health Assessment' module.



    The Search results will display eCHATs for an Individual from different linked providers amongst whom the Individual with same Oversight ID is associated .



eCHAT Unified Search for Linked Providers
  • Users of different Linked Providers will be able to access the Comprehensive Health Assessment Tool (eCHAT) of an Individual in their Caseload if the instances of that Individual in all these Providers are linked by the Oversight ID.

    Users may click on the Unified Search link of 'Comprehensive Health Assessment' module of their FirstPage in order to view the list of Individuals.



    On the 'Comprehensive Health Assessment Search' page, you may enter the first few letters of the Individual Name and an auto complete list will appear showing possible matches. Select the particular Individual Name from the list. Please note that Names of Individuals with 'Approved' eCHAT will appear in the auto complete list.



    The Search results will display eCHATs for an Individual from different linked providers associated with the same Oversight ID. Please note that users will able to view only the 'Approved' eCHAT of Individuals in different Linked Providers if the user have access to that particular Individual in their Caseload.



    On top of the Individual's eCHAT form, users will be able to view the Provider Name of the Linked Provider from where the eCHAT was initially generated.



    Comprehensive Health Assessment Tool (eCHATs) are shown for 'Admitted' Individuals only. This is to ensure that the users are unable to view eCHATs for Linked Individuals of different Linked Providers where the Individual is no longer active.
