You will notice that the text boxes have a shaded background. This means you cannot enter data here directly using the keyboard. Use the 'Look Up' button to add a Physician/Specialist or a Hospital. If your desired contact is not in the list that pops up, you can add it (to the pop-up list) by using the 'Create New Shared Contact' link near the bottom of the pop-up page.
