Custom Form is a new addition in the Therap Application, launched in Therap 8.1. This feature enables Therap with the capability of creating customized forms, based on users’ requests, with complete work flow, archiving, navigation, and security. These forms are typically used for documenting different types of assessments and special needs. When published in the Therap system, these forms will be available to users with appropriate roles like any other Therap Form.
This year in Therap 8.13, we have included two Custom Forms especially 'customized' for our New Mexico users: the DDSD Aspiration Risk Screening Tool and the Medication Administration Assessment Tool. These two forms needs to filled in before using the Electronic Comprehensive Health Assessment Tool (e-CHAT).
