Summary Reports are a collection of the data from a user's eCHAT (Electronic Comprehensive Health Assessment Tool) form. It can be created only after an eCHAT form has been approved. Approved eCHAT forms cannot be edited or updated, although they may be discarded and their data copied into a new form. The Summary Report can be generated by clicking on either the Create Summary Report link at the top or the Create Summary Report button at the bottom of the e-CHAT form.
Clicking on the button/link will take you to the Summary Report section of the Comprehensive Health Assessment page.
Clicking on the Create Summary Report link will take you to the generated report. Two columns, HCP (Health Care Plan) and MERP (Medical Emergency Response Plan) follow each item in the report. The two letters 'C' and 'R' that appear in those two columns indicate whether any followup action depends on the discretion of the user or is a requirement. Each section that requires followup action will be classified by either a 'C' or an 'R' depending on the severity of the data entered in the eCHAT form. The 'C' stands for 'Consider taking further action' and it upto users to decide whether or not any followup is needed. The 'R' stands for 'followup Required' and users must followup on the report with necessary steps. (Note: mark the column you are talking about.)
The next column is to save information about whether followup action was needed under the HCP or MERP and when it was undertaken. On completion of the action, a check box is marked to indicate whether action was taken and the date of completion of that action.
Each section has a separate 'Save' button. Therefore, instead of entering all the data and then clicking the Save button once, you may now update only the section that has new information available.
The system generates and displays the user acuity at the bottom of the report. This gives an idea about an individual's health based on all the entered data.
The report can be saved as a PDF document by clicking on the 'Display PDF' link and then saving the PDF generated.
Once a user navigates away from a Summary Report to another page, the user can view that Summary Report again by making a 'Search' for the eCHAT from which the Summary Report was generated.
