Review eCHAT

Once an eCHAT form has been approved, you will not be able to edit it. The form will be available for review, on the Review link under the Comprehensive Health Assessment area of your FirstPage or under the Health tab from your Dashboard.

Then select your required form from the list of Approved forms.

The 'reviewer' will be able to place his 'Comments', 'Create Summary Report', 'Discontinue' or 'Discontinue and Copy as Draft' an Approved eCHAT form.

Discontinue vs. Discontinue and Copy as Draft
If you 'Discontinue' a form, the form will be discontinued and will not be available for later use. If you "Discontinue & Copy" the current form it will be discontinued and 'copied' into a new, pre-populated and editable form, ready for next assesment.
