Medication Reconciliation

The Medication Reconciliation module has been introduced to meet CCHIT requirements. For more information on Therap's CCHIT Compliance Plan, please refer to the CCHIT Requirements page.

This module keeps track of an Individual's medical reconciliations and saves copy of attached Reference Medication / Reconciliation document.

To add medication reconciliation for an Individual, please follow the steps below:

1. Click on the 'Medication reconciliation' link in the 'Individual Medical Information' area of your FirstPage.
   
   
   
   
2. Select a Program from the Select Program page..
   
   
   
   
3. Next, Select an Individual from the Select Individual page.
   
   
   
   
4. Select a reconciliation type from the drop down menu, and add related details. You may add the Reference Medication / Reconciliation document to the form. Click on the 'Add Reconciliation' button to add the reconciliation.
   
   
   
   The reconciliation will be added and a success message will appear.
   
   
   
   To edit/delete an already added Medication Reconciliation, click on the Edit button.
   Click on the 'Delete' button at the bottom of the form to delete the Medication Reconciliation.
   Make necessary changes and click on the 'Update Reconciliation' button to update the Reconciliation.
   
   
   
   
5. You may add multiple reconciliations by clicking on the 'Add Reconciliation' button.
