Incident Report for Users in Montana

To enter additional information to the state form used for incident reporting in Montana, Users may add Montana Incident Report to the GER form. While entering Sate Specific information to a GER form, Consumer Incident should be defined as stated in DDP Incident Management Policy. The purpose of this policy is to establish the protocols, procedures and guidelines for the Incident Management System, within the Department of Public Health and Human Services, Developmental Disabilities Program (DPHHS/DDP). This policy complements and expands on the requirements found at ARM 37.34.1500, et seq. The policy provides structure and develops a consistent, statewide system of incident reporting, trend analysis reporting, and critical incident investigations for Developmental Disabilities Service Providers. To add Incident Report for Montana, please follow the steps mentioned below:

  1. Click on the New link under the 'General Event Reports' (GER) area on your FirstPage or Dashboard.



  2. Select the appropriate Program from the list displayed on the Select Program for GER page.



  3. Select the particular Individual from the list on the Individual List for GER page. A new GER form will open.




  4. In the Add Event area of the GER form, enter information about the event you are reporting on. Separate forms are available here for different types of events. If you do not find the event type you are looking for, click on the 'Other' button.



  5. After adding an event to the GER form, you will be able to enter additional state specific information regarding the event from the State Specific Information section.

    For example, if you have entered an Injury event to the GER form, then click on the 'Incident Report (Montana)' button under the State Specific Information section of the GER form.



  6. The Incident report (Montana) section will appear in a pop-up window where the Type of incident (Primary) will be selected as 'Injury'. Enter Description of the Incident, Supervisor Review and other additional information in this section. When you are done, click on the 'Add' button.
    2012.0 :: This feature will be available from Jan 15, 2012.
    'Cause of Incident(Primary)' section has been made required in the Incident Report for Montana.





  7. This will add the state specific additional information as the State form for the event to the GER form under the State Specific Information section.



  8. After Approval of this GER form, PDF document of the Incident Report (Montana) for Injury Event will be generated. Users will be able to access the PDF version of the Incident Report (Montana) for Injury Event from the PDF(s) section of the approved GER form.
