The Risk Management Assessment and Plan (Version : DHS-ND-2012.1) Custom form of Therap Applications is used for ND Risk Assessment. Users with the CF Approve role are able to import this custom form template from the Global Custom Form Template Library. Once the form is imported users will be able to access it form their FirstPage.
 | Please make sure that the Risk Management Assessment and Plan (RMAP) is imported only once for your Agency as the form will be available Agency wide. If you cannot find the Risk Management Assessment and Plan Section on your FirstPage please ensure that you have the required roles to access the form. We recommend contacting an Administrator at your Agency before importing the Form from the Template Library. |
Please follow the steps mentioned below to import the ND Risk Assessment (RMAP) Form from the Global Template Library.
1. Click on the Global Custom Form Template Search link under the Global Template Library area of your FirstPage.
2. On the following search page fill in your parameters and click on the Search button.
- Type in "Risk" in the Template Name/Keywords field.
- Select Published for the Status field.
3. From the list of published Custom Form Templates that matches your search parameter, select Risk Management Assessment and Plan.
4. This will open the template for the Risk Management Assessment and Plan form. Scroll down to the bottom of the Form and click on the Import button to import the form.
5. To be able to import the form, you have to agree with the Library User Agreement. Select I have read and agree with the Library User Agreement, the Website User Agreement and your Privacy Policy and click on the Continue button if you have agreed with Library User Agreement
6. A confirmation message will appear, and the Risk Management Assessment and Plan will be accessible form your FirstPage.
