Required Roles
The Health Passport is based on the eCHAT Roles. Any of the eCHAT roles will enable the user to generate a report using the Health Passport.

Requirements for a complete Health Passport report
Users will be able to generate a complete Health Passport report for an individual if they have the following information:
- An Individual Data Page (of an Individual with 'Admitted' status)
- An Electronic Comprehensive Health Assessment Tool (eCHAT) in 'Approved' status.
Generating the Health Passport
Please follow the steps below to generate the Health Passport report:
- Click on the 'Health Passport' link or the 'Generate' link under the Comprehensive Health Assessment area from your FirstPage or Dashboard.
 
- Select the appropriate Program from the Select Program page.
- Select the name of the Individual from the Select Individual page.
- The next screen will display the Health Passport report for that particular individual.
- Users will be able to generate a PDF and a Consultation Form by clicking on the 'PDF of Health Passport' link or the 'Open Consultation Form' button at the bottom of the Health Passport respectively.
- Users can also view the eCHAT form (if the Comprehensive Health Assessment form is Approved) directly from the Health Passport page by clicking on the 'Open eCHAT Form' link.
