The Developmental Disabilities Supports Division (DDSD) under New Mexico Department of Health requires all Provider Agencies to report significant events using Therap's GER Module. DDSD strongly recommends that Provider Agencies also use the GER for their internal incident reporting and management. This system gives the provider easy access to reporting and data trending for all incidents, as well as the ability to easily track notifications and follow-up. The purpose of GERs is to report, track and analyze significant events experienced by adult participants of the DD Waiver program, which do not meet criteria for abuse, neglect or exploitation, but which pose a risk to individuals served. Analysis of reported significant events is intended to identify emerging trends so that preventative action can be identified at the individual, provider agency, regional and statewide levels. The Incident Report (IR) and Medication Error Report (MER) applications have been combined in the General Event Reports (GER) application to ensure wider features and flexibility. Users can record multiple related events such as injury, medication error, death, and restraint among others. Common information on all event types needs to be entered only once. To allow more flexibility there are separate Submit, Approve, Review, and View roles for each event type. Users with the appropriate roles can create, approve, review, view, and delete these reports.
