Different Sections of the Health Passport

There are mainly five sections of the Health Passport:

  • The first section of the Health Passport contains the contact details of the individual that is pulled from the Individual Data page (ID) module.



    Users are able to view Social Security Number (SSN) of an Individual in its full form, rather than only last 4 digits.



    The information for the Immunization Date is pulled if the individual has any immunization records entered. Other than the mentioned components, The Allergies information is also populated from the Individual Data module.



  • The second section of the report provides individual's active diagnoses list which is pulled from the Medical Info section of the Individual Data module.



  • The third section of this report is populated with Physician/Specialist Contacts from the Shared Contact section of the Individual Data module and also with the contacts from the Contacts section of Individual Data.



    The Physician/Specialist Contacts section pulls the following Types of Shared Contacts from the Individual's IDF:
    • Physician
    • Specialists
    • Dentist
    • Pharmacy
    • Nurse
    • Case Manager
    • Behavior Support Consultants

The Contacts section pulls from the Individual Contacts section of the Individual's IDF depending on the following:

Emergency Contacts If the contact is selected as an 'Emergency Contact'.
Family Members If the contact has any of the following 'Relationship' - Parent, Spouse, Guardian, Relative.
Decision Makers If the contact is selected as a 'Medical Guardian'.
  • The fourth section of the report consists of check-boxes for the Major Safety Issues & Risk Factors. These safety issues and risk factors mainly come from the eCHAT and Individual Data module. Check box images are shown under 'Major Safety Issues & Risk Factors' when Health Passport PDF is generated.

    If G-Tube, G/J Tube type is selected

    J – Tube
    eChat Section 9. Tube Feeding/Enteral Nutrition
    If J-Tube or G/J Tube type is selected

    Pacemaker
    eChat Section 13a. Is a pacemaker in place
    Vagus Nerve Stimulator eChat Section 12:
    Is vagal nerve stimulator (VNS) in place
    Communication Disorder
    From Individual Data page Communication Modality
    Behavioral Issues eChat Section 19, Behavior Symptoms, if Has recent change in Behavior System" selected
    Substance Abuse If the individual is diagnosed with any substance abuse type other than tobacco use.
    Tobacco Use If diagnosed with 'Nicotine Dependence (305.10)'
    Asthma/COPD If diagnosed with any diagnosis related to Asthma or COPD.
    CPAP/Sleep apnea If diagnosed with any diagnosis related to Sleep apnea.
    Incontinent eChat Section 17: Bowel and Bladder
    If either bowel or bladder is sometimes or always incontinent

    Constipation
    eChat Section 16b:
    If has any issue with constipation
    Weight Loss eChat Section 7b. Has there been unplanned weight loss

    Weight Gain
    eChat Section 7a.
    Has there been unplanned weight gain (> 5 lbs)

    Heart Disease
    eChat Section 13. Cardiac/Circulatory
    Is there a known cardiac or circulatory condition
    Fall Risk eChat Section 24. Falls
    Hearing Aid(s) eChat Section 22 Known hearing impairment
    If either Uses aide(s) , Refuses aide(s) is selected

    Glasses/Contact Lenses
    eChat Section 22 Known visual impairment
    If either "Uses glasses or contacts" or "Refuses glasses or contacts" is selected
    Walker/Cane/Other From Individual Data page Mobility
    Wheelchair From Individual Data Mobility
    Dentures/Partials Currently there is no mapping
    Assistive Technology Currently there is no mapping



  • In the final section the report consists of the Advanced Directive , DNR Order sections (auto-populated from the Medical Info section of Individual Data) and Other Important information(auto-populated from the Other Comments section of the eCHAT Section 28 ). The Signature and Date is to be entered manually once the report has been printed out.



  • Users may also generate a PDF and a Consultation Form from the Health Passport by clicking on the 'PDF of Health Passport' link or the Open Consultation Form button at the bottom of the report.
