Create New Medication History

Users assigned with the HT Submit role are able to create Medication History forms. In order to create a new Medication History form, please follow the steps mentioned below:

  1. Click on the New link below the Medication History column header in the Health Tracking area of the FirstPage or the New link in the Medication History section after going to the Health tab of the Dashboard.





  2. On the Medication History form, select the Program Name first and then select the Individual Name from the drop down lists. You may click on the Medications link in Look up to view all the Medications that were assigned to the Individual. You may also click on the Allergies link to view the Allergy Profile of the Individual.

    Selecting the Program Name will display the names of the Individuals enrolled in the Program for selection from the Individual Name drop down list.








  3. In the next section, you may enter the first few letters of the Drug Name and click on the 'Search' button. This will display a list of drugs that match the letters entered. You may then select the particular drug from the list.



  4. You may also type in the Drug Name and select the appropriate drug from the auto complete list that appears.



  5. You may click on the 'Drug Name' and view the Drug Description, Side Effects and other information.



  6. Fill in the 'Medical History Information' section with details of the medication and schedule.
For CCHIT Compliance, Home Medications can now be recorded and displayed in the Medication History form. A "Home Medication" is any prescription or over the counter medication that is owned by the Individual and brought into a facility such as a respite center, nursing home, hospital, etc, that were not ordered for that person by an attending physician or other prescriber on duty at that facility.





  1. You may precisely schedule a medication in the new Medication Schedules section with advanced options like 'Day of the week', 'Every Alternate Day' etc. If the Frequency, Starting From date and the Schedule Time of a Scheduled medication is entered, the corresponding cells in the MAR will be automatically colored green.



    To schedule medications in an odd pattern, you may only select the 'Schedule Time' from the 'Medication Schedules' section. You may then schedule medications randomly or in an odd pattern from the MAR Configuration page. For further details regarding this, please click here

  2. Fill in the rest of the information in the remaining fields as necessary. You may click on the 'Look Up' button beside the Prescriber field in order to select a prescriber from the Shared Contact list.





  3. Select the schedule time for the medication. Click on the 'Add Another Schedule Row' button to add further schedule rows to the form.



  4. Click on the 'Save' button to save the form in your Worklist or click on the 'Submit' button to save and submit the Medication History form.



  5. The system will check the interaction of the new Medication with existing active Medications and Allergies of the Individual. In case a negative interaction is found a warning message will be displayed.
    • Click on the 'No' button to get back to the Medication History page.
    • Clicking on the 'Yes' button will save the new Medication despite the allergy and/or drug interaction.



      Any Drug/Allergy interaction with a saved or submitted medication will be displayed as a warning in the Medication History form.


  6. A success message will be displayed on the next page once the Medication History form has been submitted. You may click on the Copy current Form link to copy the contents of the form for creating another Medication History form. You may also click on any of the Health Tracking links located at the bottom of the page to create a corresponding new form.
