Individual Care Plans are Individual based forms. One Care Plan is used for a single Individual. To create Care Plans for multiple Individual's, please create separate Care Plans for each of the Individual.
To create an Individual Care Plan for a specific Individual, please follow the steps mentioned below:
1. Click on the 'New' link under the 'Individual Care Plan' module on your FirstPage or Dashboard.
In the Dashboard, the Care Plan module is under the Health tab.
2. Select a Program
3. Select an Individual. This will open the New Care Plan page.
4. Describe a problem in the 'Problem' field and add Goal(s)
5. Click on the 'Add' button to add an approach.
6. You may add team meeting notes by clicking on the 'Add' button under the 'Team Meeting Discussions' section
7. Add team meeting note and click on the 'Save' button.
8. At the bottom of the page, click on the 'Done' button to keep the Individual Care Plan as a draft.
- Click on the 'Submit' button to submit the form for approval.
