An Allergy Profile option has been added in the Individual Medical Information module. This option enables users to document an Individual’s allergy information in detail. This information is used to determine if the medications in the Medication History forms of an Individual have any interactions with the Individual’s active allergy(ies). Providers with access to First DataBank will be able to view the Drug-Allergy interaction along with Drug-Drug interaction information for the Individuals.

To Create an Allergy Profile for an Individual, please follow the steps below:
- Click on the Allergy Profile option in the Individual Medical Information section of the FirstPage or from the Individual tab in the Dashboard.
- Select the Program and the Individual for whom the Allergy Profile is going to be created.
- After selecting the Individual, an Allergy Profile for that Individual will be opened where you can add allergy information by clicking on the 'Add Allergy Information' button.
- Clicking on the 'Add Allergy Information' button will take you to the page where you can add 'Allergy Detail Information' for that Individual.
- In this page, you may type in the first few letters of the allergy name in the Allergy field and look-up for that allergy by clicking on the 'Search' button. Access to First DataBank will provide the list of allergies that match with the typed in letters.
- If you do not find the required allergy name from the list of Allergies, you may type the name in the +Allergy6 field in the Allergy Detail section.
 | A Warning message will be displayed showing any possible interaction between the newly added allergy and the active medications of the Individual.

 | Drug-Allergy Interaction will take place for looked-up allergy data only. The allergies that were not found in the list of Allergies will be added in the Active Allergies section with a different color for ease of identification. |
- You may type in the code for that allergy in the Code field and select the Coding System from the drop-down list.
- You may add a description for the allergy in the Description field.
- You must select an allergy type from the Type drop-down list.
- You may select the level of severity for the allergy, the date when the allergy was identified and the reactions noted for the allergy.
- After entering all the information, click on the 'Save' button to save the allergy details.
- The 'Allergy Detail Information' will appear in the Active Allergies section of the Individual’s Allergy Profile.
- Clicking on the name of the allergy will take you to the Allergy Detail Information page.
- If any active allergy has an interaction with any active medication, then this information will be displayed in the Allergy Profile page.
- In the Allergy Detail Information page, you can update the allergy details, deactivate or delete the 'Allergy Detail Information'.
- Deactivated allergy details will appear in the Inactive Allergies section and deleted allergy details will appear in the Deleted Allergies section of the Individual’s Allergy profile.
- You may delete an Inactive Allergy Information, by clicking on the name of the allergy in the Inactive Allergies section. This will take you to the Allergy Detail Information page where you need to click on the 'Delete' button after adding the reason for deletion.
- Any allergy related comments can be added in the Comment section.
- If an Individual has no allergies or no drug allergies then you may select the No Known Allergy or No Known Drug Allergy check-boxes respectively and click on the 'Save' button.
- Information from the Allergy Profile is checked by the system when Medication Histories are created for the Individual.

 | Saving comments in the Comment textbox removes the previous comment. They, however, may be viewed from the Update History link at the top of the page. |
